Patient Information

New patients are required to fill out a New Patient Details form prior to their visit to the doctor, please arrive early to allow for this. Alternatively, if you have access to the pre-consult questionnaire through BetterConsult, you may also pre-fill this information.

All new patients are booked a double appointment with the doctor to allow enough time to gather relevant clinical information; including medications, past medical history and general social history.
We may also ask that your fill in a form to transfer your previous medical records to our clinic.

The doctor will advise you as to how to obtain your results. You will be asked to ring us or you will be contacted by us should your doctor wish to see you about the results. If you have any concerns about your results please don’t hesitate to contact us.

This confidential document is a valuable resource in your long term medical care. In keeping with modern practice, the information is held electronically. A great deal of effort is made to keep the record accurate and complete. Procedures to keep the record secure from unauthorised access, theft and physical loss e.g. fire are in place.

A patient health summary is available free at any time and patients are encouraged to ask for this; please update us on any errors. More detailed reports can be provided for a fee. Referrals to other health professionals including specialists and allied health include your past medical history. Please let us know if you wish for a specific condition to be withheld.

From time to time, the doctors at Sapphire Clinic may take part in research and educational activity which involves presentation of de-identified medical records. We will always obtain your consent if any research involving de-identified patient data is supplied to organisations outside the practice.

During practice accreditation, an external GP will select and review a sample of records in order to assess the quality of record keeping. Please notify us if you wish to have your record excluded from this inspection.

Your medical records are confidential. It is the policy of this practice to maintain security of personal health information at all times and to ensure that this information is only available to authorised members of staff.
